On the first question, as a start, you could look at the Judgment of the
International Court of Justice in the North Sea Continental Shelf cases, ICJ
Rep. 1969, which includes a discussion of this issue. There is a lot of
literature commenting on this, amongst other things.
On the second question, you might like to look at the International
Hydrographic Organization's "Manual on Technical Aspects of the UN
Convention on the Law of the Sea 1982", though this is not dispositive.
