I am going to disobey & offer a partial answer. Poetry in the USA started
going to this kind of hell-in-a-handcart long since. Lyn Hejinian told me
many years ago that in the early 70s there was _The Cunt Coloring Book_ ,
offered by the Down There Press.
"learning to give blow jobs" as a poetic exercise immediately dates back to
Gary Snyder's "How to Make [forget the adjective] Stew in the [forget the
name] Desert", with its notion that useful k-edge might also be a poem. And
of course, along another track, to the Peter Orlovsky piece describing the
blowjob Ginsberg is providing while he, P.O., balances the tpywriter on
A.G's back.
The "stomach exploding" poem/s likely derive from a Monty Python movie
scene. So back to you on that one, Britain!
"a Barbie dipped in shit" has a CW ring to it, and tempts one to supply
further lyrics, for a David Lynch movie:

As to the crust theme, what I wonder would the expression "he's got a lot
of crust" mean in its context? High Testosterone??
