For the twelfth century and later, the _locus classicus_ is
in part III of Gratian's _Decretum_ (De consecratione),
distincio 1, canons 19 and 20. Although both of these
canons make claims of antiquity, the first cannot be traced
back later than the tenth century and the second appears to
original in or near the _Decretum_ itself.
I wrote a paper on this very topic some time back, and am
in the middle of rewriting it for (hopeful) publication.
