I have seen a pictures of a burial in just a shroud in a book of hours
at the University of Kansas.

I believe it was fairly common for people to be buried in just a shroud,
particularly if you were unimportant.
One of the troubles with gardening in our churchyard (closed in 1850, but
probably with 1000 years of burials) is that various bits of long-dead
people work their way to the surface. I just dig as deep a hole as possible
and re-inter the bits with an apology and a prayer. I have also walked up a
churchyard path which had had the banks either side cut back. This had
exposed a number of bones which many people found unacceptable.
